KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across the state of Kentucky and potentially contiguous states. The goals of the Home-Study Post Release Services Program (HS/PRS) are to: Assess the safety and suitability of a sponsor before an unaccompanied child is released into a sponsor’s care (Home Study). Provide case management support to ensure the continued safety and stability of children in sponsor’s homes and linking children to resources in the community (Post Release Services). The PRS Intensive Case Manager (ICM) provides intensive case management services for families needing additional support. Their primary focus is stabilizing the family unit through tailored interventions and connecting the released child and sponsor family to community-based services for long-term care. PRS ICMs deliver Level 3 Post Release Services and may also provide Level 2 and Level 1 services. This role requires flexibility to work outside standard business hours, including evenings and weekends, as the HS/PRS program operates on a 24/7 schedule. Team members coordinate schedules to meet client needs, with approval from the Program Director or PRS Case Management Supervisor.
