About The Position

The Post Kidney Transplant Social Worker will coordinate and facilitate social work services upon identifying or notification of patient requiring services, both outpatient and inpatient. Coordinate cross coverage with the pre-transplant social worker to ensure continual social work coverage for the transplant center. You will evaluate financial needs for assistance with medications in the post-transplant population. Work with pharmacies, insurance companies and grant programs to ensure patients maintain their medication regime. You will communicates effectively with transplant staff, hospital staff, physicians, dialysis center staff and patients Intervenes with problems identified with the appropriate resources to include psychosocial needs of patients and their families. Lead patient support groups for patient focused issues in the post-transplant population. You will function as a member of the multidisciplinary team. Document appropriate information in the patient's medical record.

Requirements

  • 2 years experience as Social worker in Hospital setting required
  • Masters in Social Work is Required

Nice To Haves

  • 3 years' experience as Social worker with 1 year in dialysis unit setting is preferred
