The Post Adoption/Permanent Guardianship Care Coordinator is responsible for demonstrating strong communication and fostering positive relationships between Prevention/Diversion staff, Department of Children and Families, Post Adoptive Families and FPOCF. In this role, the liaison connects youth and families to essential community resources and facilitates family team conferences to ensure coordinated support. This position will act as the primary point of contact for post-adoption families in need of care coordination. This position will work to meet the needs of families, which may include evenings/weekends. The care coordinator will actively participate in, and facilitate, as needed, Local Review Teams and other applicable staffings/meetings. This care coordinator will act as the primary point of contact between the Seminole dependency court system and FPOCF in all cases involving closed Permanent Guardianship. Working in conjunction with the courts and FPOCF operations and diversion staff, this position will develop and manage a process to ensure that inactive cases involving permanent guardians continue to be stable and have access to any needed services. They will document activities and actions in Florida Safe Families Network (FSFN). Additionally, they will maintain data for reporting purposes, to identify gaps in services, for resource allocation, and for forecasting needs.
