JOB DESCRIPTION AND POSITION REQUIREMENTS Scheduled Weekly Hours: 40 WORK SHIFT: Day shift, primarily Monday-Friday with about 4 weekend shifts per 6-week schedule JOB SUMMARY: Coordinates the post-acute plan discharge arrangements including post-acute care and Durable Medical Equipment (DME) set up. Provides logistical support to ensure a safe and timely discharge for patients to include but is not limited to patient discharge disposition and arrangements, post-acute discharge planning, and coordination of discharge activities.

Requirements

• Bachelor’s Degree or higher in Social Work and one year healthcare experience in an acute care setting preferred.
• Required: LA LPN license, LA Social Work license, or Medical Assistant Certification
• Ability to read, understand, and follow through on written instructions.
• Knowledge of multiple software programs, including Microsoft Work, Excel, and Internet.
• Familiar with office machines and equipment, such as copier and fax machine.
• Able to read medical terminology and assemble results from medical record.
• Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
• Highly organized and efficient in establishing priorities and performing tasks within specific timeframes.
• Self-motivated requiring minimal supervision.
• Must possess good physical health.
• Some requirements include but are not limited to standing, sitting or walking for long periods of time.
• Lifting at least 10 pounds is required.
