JOB DESCRIPTION AND POSITION REQUIREMENTS Scheduled Weekly Hours: 40 WORK SHIFT: Day shift, primarily Monday-Friday with about 4 weekend shifts per 6-week schedule JOB SUMMARY: Coordinates the post-acute plan discharge arrangements including post-acute care and Durable Medical Equipment (DME) set up. Provides logistical support to ensure a safe and timely discharge for patients to include but is not limited to patient discharge disposition and arrangements, post-acute discharge planning, and coordination of discharge activities. MINIMUM QUALIFICATIONS: Bachelor’s Degree or higher in Social Work and one year healthcare experience in an acute care setting preferred. Required: LA LPN license, LA Social Work license, or Medical Assistant Certification Ability to read, understand, and follow through on written instructions. Knowledge of multiple software programs, including Microsoft Work, Excel, and Internet. Familiar with office machines and equipment, such as copier and fax machine. Able to read medical terminology and assemble results from medical record. Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity. Highly organized and efficient in establishing priorities and performing tasks within specific timeframes. Self-motivated requiring minimal supervision. PHYSICAL DEMANDS: Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 10 pounds is required.
