About The Position

BJC Medical Group Post Acute Care is seeking a skilled and compassionate physician to join our team on a PRN basis. In this role, you will provide high-quality medical care to patients in skilled nursing facilities across Missouri and Illinois, rounding on an average of 10–12 patients per day between 8:00 AM – 5:00 PM with scheduling flexibility at provider discretion. Role Purpose The physician will provide professional medical services within the practicing Specialty to the best of physician’s ability through direct patient care and spend additional time as necessary to perform other related duties such as completing medical records, providing MyChart consultations and inbasket management, conducting patient-specific education and collaborating with advanced practice providers and care team members. Requirements

  • MD or DO with a valid, unrestricted medical license in Missouri and/or Illinois
  • Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics
  • DEA certification and current CPR/BLS required
  • Strong clinical acumen, sound decision-making, and excellent communication skills
  • Collaborative, team-oriented approach with adaptability to changing patient needs and facility environments
  • Comfort with EHR platforms used in SNF settings, including EPIC
  • Doctorate - Medicine

Responsibilities

  • Coordinate scheduling and maintain regular communication with the BJC Medical Group PAC Practice Administrator and APP staff.
  • Conduct comprehensive medical assessments, evaluate new admissions, follow up on existing patients, and address acute issues as they arise
  • Diagnose and manage acute and chronic medical conditions, review and place orders for medications, treatments, labs, imaging, and other diagnostics
  • Collaborate with nursing staff, rehabilitation teams, social workers, and APPs to develop cohesive care plans; attend IDT meetings as scheduled
  • Communicate medical findings, care plans, and changes in condition to patients' families or healthcare proxies in a timely and compassionate manner
  • Complete accurate, timely documentation — including H&Ps, progress notes, and discharge summaries — in EPIC and facility-specific EMRs
  • Ensure adherence to HIPAA and all applicable federal, state, and organizational protocols
  • Manages the medical care of patient panel by providing or otherwise arranging for inpatient hospital care of physician’s patients, either through regular hospital rounds, making arrangements with one or more hospitalist(s) or other qualified physician to provide coverage for physician’s hospitalized patients consistent with Medical Staff requirements.
  • Collaborates with patients, families, and members of the care team to ensure excellent patient care outcomes at the clinic location(s) designated by BJC and any other BJC clinical outreach location to which physician may be assigned as patient care demands.
  • Performs and documents medical histories and physicals in the patient’s medical record as required by hospital medical staff bylaws.
  • Provides or arranges for call coverage for clinic patients and inpatient call coverage in a manner acceptable to BJC and in accordance with Medical Staff bylaws, while observing and following all BJC policies and procedures and all applicable legal, ethical and professional standards.
  • Collaborates and teaches advanced practice providers, support staff or any care team member assigned in the care of physician’s patient panel.
