Mountain View Hospital is looking for a Population Health Management Supervisor to join our team! JOB SUMMARY: Oversees daily operations of the population health management team to ensure smooth operations and to provide patients with the best possible care experience from Mountain View Organization. Requirements

RN degree with BLS certification
6 months required experience
Must be able to use Microsoft Office, WORD, Powerpoint, Excel, Microsoft Office, and various EMR systems
Must have the sensitivity, maturity, and ability to communicate clearly and concisely, both verbally and in writing
General mathematics
Can concentrate for long periods of time in order to complete assigned tasks with a better than average attention span in order to listen, read and remember verbal and written instructions
Can understand and act upon input from the manager concerning errors and instructions
Ability to instruct and work well with others as part of a team
Full time position

Nice To Haves

2-5 yrs clinical experience preferred
