About The Position

Collaborate with team members to improve health outcomes for high-risk populations. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Cross reference clinical databases to identify high-risk populations Participate in quality data collection for programs focused on improving health outcomes Communicate with clinical team regarding high-risk population needs and engagement Facilitate connecting high-risk populations with appropriate community resources. Document interactions and collaborative efforts in electronic medical record Help with needed wellness services for clients when requested Assist with staff wellness when requested Attend required clinical meetings prepared with information to advocate and benefit high-risk populations Attend staff meetings, in-service training, workshops, and seminars and adhere to agency policy, rules, regulations, and procedures Performs other duties as required

Requirements

  • High School Diploma or General Equivalency Diploma required
  • Minimum two years relevant experience required
  • Oklahoma Department of Mental Health and Substance Abuse Certified Case Manager I, if eligible, and Wellness Coach required within 90 days of hire required (training provided after hire)
  • Valid OK driver’s license, clean driving record, reliable transportation required
  • Excellent verbal, written and electronic communication skills required
  • Commitment to the mission of Red Rock Behavioral Health Services
  • Must have the ability to stand or sit for long periods of time
  • Must have the ability to lift, push, or pull a minimum of 25 pounds
  • Ability to travel approximately 10% of the time
  • Ability to use telephone, PC, fax machine, copy machine, and printer

Nice To Haves

  • Bachelor’s Degree in mental health or health-related field preferred.
  • Peer Recovery Support Specialist certification, if applicable, preferred

Responsibilities
