About The Position

Erie Family Health Centers, a top workplace in Chicago and the suburbs, is looking for a valuable addition to our Patient Program and Support Services! The Population Health Navigators at Erie connect patients with a range of services to address the social drivers of health. This includes linking patients with internal resources at Erie like behavioral health or case management services as well as connecting patients to local community resources such as rent/utility assistance, food programs, transportation, and legal aid. The Population Health Navigator works closely with patients to understand their unique needs and curate individualized resources to overcome barriers.
