Requirements

  • Applicants should have a state of Michigan RN license and possess an associate degree from an accredited nursing school (bachelor's degree preferred).
  • Basic Life Support (BLS)
  • Two years healthcare experience required
  • Core Competency in risk adjustment preferred
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Responsibilities

  • Manages the department's activities and resources to drive quality outcomes.
  • Reviews Electronic Medical Record (EMR) and payor registries to identify high risk patients.
  • Analyzes clinical data and identifies Hierarchical Condition Category (HCC) gaps.
  • Queries/communicates with providers/care managers to capture accurate Hierarchical Condition Category (HCC).
  • Assists in the evaluation of data collected to assure accuracy and reliability.
  • Prepares reports and performance reporting of quality data.
  • Oversees quality reporting to the insurance carriers.
  • Assists with orientation and training of personnel and students.
  • Participates and attends meetings as required and/ or assigned.
  • Performs other related duties as assigned or requested.
  • Prepares periodic reports as requested.
  • Assists Care Managers in making follow-up calls to patients.
