About The Position

The Population Health Manager at CVS Health is responsible for enhancing healthcare quality and achieving ACO Shared Savings by managing performance metrics, serving as a project manager, and training partners on foundational healthcare programs. This role involves building strong relationships with primary care practices, facilitating communication, and identifying opportunities for improvement in healthcare delivery.

Requirements

  • Minimum two years of experience in provider clinic, health system, account management, partner services, or consulting.
  • Bachelor's degree from an accredited university or college is required.
  • Valid RN License preferred.

  • Previous experience in project management.
  • Previous experience working with Accountable Care Organizations.
  • Residing in or near Dallas, TX preferred.

Responsibilities

  • Manage key performance indicators for assigned primary care practices.
  • Coach partners on Foundational Programs including patient messaging and annual wellness visits.
  • Conduct monthly Action Planning calls with partners to support population health priorities.
  • Provide project management and oversight for partner ACO work.
  • Serve as the first escalation point for partner questions and concerns.
  • Communicate business and technology issues and solutions effectively.
  • Build and maintain strong relationships within CVS Accountable Care and partner organizations.
  • Integrate population health software into partner interactions and optimize its utilization.
  • Gather data and track performance metrics to drive discussions on improvement opportunities.
  • Document partner progress and connect them with subject matter experts as needed.
  • Promote resources provided by subject matter experts on quality reporting and ACO compliance.
  • Attend regional huddles and partner events to support initiatives.
  • Identify and promote training content related to specific partner needs.
  • Hold partners accountable to goals related to healthcare quality and ACO shared savings.
  • Stay current on evolving ACO program requirements from CMS and CVS Accountable Care.
