About The Position
Responsible for identifying population health needs and facilitates best practice service delivery to achieve healthy lifestyle and better health outcomes for the population.
Responsibilities
- Identifies population health needs, map services and identifies gaps in healthcare services.
- Engages relevant stakeholders (internal and external) to develop and implement service delivery requirements.
- Establishes and maintains strategic and program alliances with other agencies whose activities impact public health.
- Facilitates and or contributes to models and plans meant to optimize capacity to meet the healthcare needs of the region.
- Collates and analyzes population data from various databases including state and national to support local planning
- Utilizes routinely and purpose-specific collected data to describe and summarizes the health status of the population to identify priorities for public health action.
- Other duties as assigned
- Travel is required to physician practices
