About The Position

We are looking for a POPULATION HEALTH CONSULTANT who can help shape our vision and support our mission. The consultant supports daily operations related to program execution and evaluation under the direction of the Population Health Director, ensuring that programs are designed to improve health engagement by integrating social determinants of health. Supports the development, coordination, and implementation of population health programs across lines of business, focusing on promoting whole-person care for equitable health outcomes and improved operational efficiencies. Manages day-to-day execution of population health programming with independent decision-making, judgment, and discretion with execution. Coordinates with relevant internal departments and external stakeholders to ensure collaborative and efficient operations. Analyzes population trends, identify best practices for subpopulation interventions, and ensure analyses address potential unintentional biases to support equitable outcomes. Conducts ongoing research and serve as a subject matter expert on public health trends in COA community, member population data, and population health programs, offering guidance to care teams, intervention leads, and organizational leadership. Applies a public health lens to programs and services with a focus on prevention and wellness. Partners with contracted vendors for delegated population health programming, as appropriate, ensuring deliverables are achieved and standard processes are followed. Develops and oversee detailed project plans, ensuring timely completion, adherence to budgets, and alignment with organizational goals. Works with cross-functional teams to align population health efforts, ensuring programs and services effectively address the needs of diverse populations. Serves as a resource on member population demographics, utilization, and health outcomes, to support tailored interventions for members, provider-facing work, and community-based initiatives. Applies quality improvement (QI) tools and collaborate with the QI department to monitor care effectiveness and identify areas for improvement. Serves as a subject matter expert on internal policies, state and federal mandates, government contract requirements, and NCQA guidelines. Educates staff to ensure consistent adherence to program standards and identify opportunities for process improvement. Conducts population-level analyses to inform risk tiering, member acuity, and population health trends. Assists in developing metrics that align with population health priorities, providing a strategic lens for how these metrics can drive equity-focused outcomes. Conducts research on population health trends to inform program initiatives, as requested. Tracks and monitors program participation and member outcomes to assess the efficacy of programs and interventions and make recommendations for improvement.
