About The Position
The Population Health Administrative Assistant is a temporary salaried position at Santa Rosa Community Health (SRCH) that plays a crucial role in supporting the organization's quality goals through various administrative and analytical tasks. This position is expected to last from October 2024 to April 2025 and reports directly to the Population Health Director. The assistant will be involved in data analysis, project tracking, and outreach efforts aimed at improving the quality of care provided by SRCH. In this role, the Population Health Administrative Assistant will actively participate in Quality Committee meetings at both the Vista and Caritas campuses. They will collaborate with campus leadership to plan and implement strategies that align with the organization's quality improvement initiatives. The assistant will analyze data to identify gaps in quality and population health needs, presenting findings and recommendations to internal committees and workgroups. Additionally, they will create outreach lists to facilitate patient engagement and education, helping to achieve the organization's quality goals. The assistant will also coordinate events such as the Mammogram Van and other Breast Cancer Screening initiatives across SRCH campuses, ensuring effective collaboration with various sites. Other responsibilities include assisting with data reporting and tracking, completing Patient Specific Alert (PSA) abstraction from electronic health records, and supporting the Partnership Patient Assignment Workgroup. The role may also involve facilitating other workgroups and committees related to quality improvement and providing project coordination for various quality improvement projects as needed. Overall, this position is integral to the success of SRCH's quality improvement efforts and requires a proactive approach to data analysis, communication, and collaboration with diverse teams.
