About The Position

We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our healthcare team as a PM Shift Unit Manager. This role is integral in providing exceptional care in a healthcare or public nursing setting. The ideal candidate will have a passion for resident care, supervisory experience, and strong communication skills.

Requirements

  • Graduate of an accredited school of nursing.
  • Active RN or LPN license in the State of Wisconsin.
  • Strong knowledge of healthcare or public nursing practices.
  • Excellent communication and leadership skills.
  • Ability to perform physically demanding tasks such as heavy lifting and extensive bending.
  • Organizational skills with the ability to multitask in a fast-paced environment.
  • Sufficient visual and hearing abilities for comprehensive communication and resident care.

Responsibilities

  • Conduct walking rounds to engage with staff and residents, addressing any incidents or concerns.
  • Review 24-hour or 72-hour reports, preparing responses for the Director of Nursing (DON) regarding critical issues such as behaviors, incidents, weight loss, or bowel movements.
  • Participate in daily interdisciplinary team (IDT) meetings, perform Root Cause Analysis, update care plans, and document in real time.
  • Manage follow-up tasks from morning meetings to ensure timely completion.
  • Attend and contribute to care conferences for assigned residents.
  • Regularly round the unit to ensure care efficiency, resident cleanliness, and overall environment orderliness.
  • Complete assessments for the Minimum Data Set (MDS) and update care plans based on resident changes.
  • Collaborate with the wound physician for weekly wound rounds and follow up on wound care.
  • Ensure compliance with walking, ROM programs, repositioning, and toileting care plans, and audit documentation accordingly.
  • Address family concerns and follow up for resolution.
  • Assist with staffing adjustments in case of call-ins to ensure adequate coverage.
  • Participate in on-call rotations as required by facility policy.
  • Oversee unit nurses and aides, ensuring care plan compliance, providing staff education, and collaborating with the DON and HR on corrective actions when needed.
  • Maintain clear communication with the team and provide reports to the DON.
  • Collaborate on resolving issues, contribute to Root Cause Analysis, and promote a positive work environment.
  • Perform other duties as assigned.
