Ensures coordination and continuity of patient care for neonatal through geriatric patients who have long-term and/or difficult vascular access needs in order to maintain organized and efficient care and provide support to staff and other health care professionals. Provides clinical management including patient/family education to achieve desired patient outcomes. Provides consultation, direction, and education related to assigned patient population to nursing staff and other health care providers. Responsible for identifying the patient's alterations in health; assessing the patient's response to treatment protocols; communicating appropriate data to staff; implementing nursing interventions based upon established treatment protocols; and anticipating potential outcomes for patients at risk for complications. Must demonstrate ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs and provide care according to protocols and within practice of nursing.
