Requirements

• M.D. or D.O. graduates.
• Board certified/eligible in Internal Medicine or Family Medicine.
• Active, non-probationary state medical license.

Nice To Haves

• Fellowship training in Geriatrics.
• Experience in caring for elderly or medically underserved populations.

Responsibilities

• Provide outpatient primary care to senior patients.
• Spend quality time with patients to build relationships.
• Address both medical and social needs of patients.
• Collaborate with care teams to ensure patient-centric care.
