Requirements

• M.D. or D.O. graduates.
• Board certified/eligible in Internal Medicine or Family Medicine.
• Active, non-probationary state medical license.

Nice To Haves

• Fellowship training in Geriatrics.
• Experience in caring for patients with comorbid conditions.
• Passion for working with the elderly and medically underserved.

Responsibilities

• Provide outpatient primary care to senior patients.
• Spend time with patients to ensure comprehensive care.
• Lead care teams with a patient-centric orientation.
• Address medical and social needs of patients.
• Build relationships with patients through small panel sizes.
