Requirements

  • Must be a licensed physician.
  • Experience in primary care and skilled nursing facilities preferred.
  • Ability to work flexible shifts and manage a caseload effectively.
  • Familiarity with electronic health record systems.

Nice To Haves

  • Experience with Theoria’s proprietary EHR system, ChartEasy™.
  • Previous experience in chronic care management.

Responsibilities

  • Serve as the attending physician for the residents residing at each facility.
  • Collaborate with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility.
  • Provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs.
  • Diagnose and treat patients with acute illness and chronic diseases.
  • Complete provider documentation in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™.
  • Participate in Theoria’s Chronic Care Management, Transitional Care, RPM and other medical quality initiatives and programs.
  • Supervise collaborating nurse practitioners and physician assistants.
  • Utilize electronic/online medical tools such as EHR systems and state/county medical systems.
