We are seeking physicians in our Green Bay, Madison and Wausau service areas. Oak Medical is a physician-founded medical group dedicated to delivering high-quality care to seniors in post-acute and senior living communities across Wisconsin. Our physicians practice where their expertise matters most — supporting residents, care teams, and families while improving outcomes and quality of life. We follow a Collaborative Care Model. You’ll work alongside experienced advanced practice providers, nurses and facility care teams to deliver coordinated, comprehensive care.
