About The Position

A physician-led organization is seeking a Board-Certified/Board-Eligible Physician (MD/DO) in Internal Medicine or Hospitalist to provide care within Skilled Nursing Facilities (SNFs) and nursing homes. The focus of this role is on improving patient outcomes and reducing avoidable hospitalizations . This is a unique opportunity for a physician who enjoys clinical practice but is also interested in transitioning into a leadership and supervisory position as the territory develops. About the Role: This position begins with a strong focus on direct patient care in post-acute and long-term care settings. Over time, as the territory grows, the physician will take on more administrative and leadership responsibilities , including oversight of Nurse Practitioners (NPs) and Physician Assistants (PAs).

Requirements

  • MD or DO degree with Board Certification/Eligibility in Internal Medicine or Hospitalist .
  • Active, unrestricted Florida medical license (or eligibility).
  • Prior experience in Skilled Nursing Facilities, long-term care, or post-acute medicine preferred.
  • Strong clinical decision-making with a focus on reducing unnecessary hospital admissions.
  • Excellent leadership, communication, and collaboration skills.

Responsibilities

  • Provide comprehensive care for patients in Skilled Nursing Facilities and nursing homes.
  • Manage chronic conditions and acute changes in status to help prevent rehospitalizations.
  • Conduct rounds, coordinate with facility staff, and communicate with families and caregivers.
  • Collaborate with interdisciplinary teams including nurses, therapists, and case managers.
  • Ensure quality of care and compliance with all state, federal, and facility regulations.
  • As territory develops: supervise and mentor advanced practice providers (NPs and PAs).
  • Support safe discharges and continuity of care through effective care transition planning.
