About The Position

Schodack Internal Medicine and Pediatrics, part of Community Care Physicians (CCP), is a well-established primary care practice providing comprehensive care for patients of all ages in a welcoming, team-based environment. Our collaborative care team includes a board-certified pediatrician, board certified Med/Peds physicians, nurse practitioners, and physician assistants dedicated to deliver coordinated, high-quality care CCP is the Capital Region’s first and largest independent physician-led medical group, serving the community for over 40 years. We offer the autonomy of private practice with the support of a strong network, along with competitive compensation of $275,000, fully paid health insurance, and comprehensive benefits. We are seeking a full-time Physician committed to compassionate, evidence-based primary care. Requirements

MD or DO degree from an accredited medical school.
Valid and unrestricted New York State medical license (or eligibility).
Strong clinical, diagnostic, and communication skills.
completion of Doctor of Osteopathy (DO) or Doctor of Medicine (MD)
completion of an accredited internship and residency
an active license to practice medicine in NYS.
Must be currently board certified by an American Board of Medical Specialties (ABMS) Member Board or an approved American Osteopathic Association (AOA) specialty board.

Responsibilities

Provide high-quality clinical care to patients of all ages in internal medicine, family medicine, or Med-Peds practice.
Perform comprehensive history and physical examinations, develop assessment and care plans, and coordinate treatment with care team members.
Manage acute and chronic conditions, including preventive care and health maintenance.
Provide same-day sick visits, wellness visits, chronic disease management, and age-appropriate screenings.
Collaborate with nurse practitioners, physician assistants, nursing staff, and administrative personnel to ensure timely, patient-centered care.
Utilize electronic health records and digital tools to support continuity and access to care.
Participate in quality improvement initiatives and practice performance activities.
Educate and counsel patients and families to promote health, wellness, and disease prevention.
