We are seeking mission-driven Physicians who are eager to be foundational members of this new territory—clinicians who want to help shape operations, build meaningful community partnerships, and deliver best-in-class care to patients across the region. Requirements

Active and unrestricted Missouri medical license (or ability to obtain)
Commitment to high-touch, relationship-centered primary care
Ability to work autonomously while collaborating within an interdisciplinary framework

Responsibilities

Provide comprehensive primary care through in-home and senior-community visits.
Serve as a Bloom ambassador in the St. Louis market, helping build brand presence and supporting market development strategies.
Conduct market analysis and engage with local care partners to understand the community landscape and identify opportunities for growth.
Manage and grow an empaneled patient population, typically completing 10 patient visits per day.
Build trusted relationships with assisted living communities, hospitals, clinics, and community organizations to strengthen care coordination.
Collaborate closely with an interdisciplinary team—including specialists, pharmacists, care managers, and social workers—to address complex patient needs.
Utilize population health data and condition-specific care pathways to improve outcomes.
Participate in ongoing internal education and clinical development programs.
Travel occasionally to Denver for onboarding and ongoing training.
