Now Recruiting for Our Newest Market: St. Louis, Missouri As part of our strategic growth and our mission to reach more patients in need of high-quality, in-home primary care, Bloom Healthcare is thrilled to be launching services in the St. Louis market. We are seeking mission-driven Physicians who are eager to be foundational members of this new territory—clinicians who want to help shape operations, build meaningful community partnerships, and deliver best-in-class care to patients across the region. Requirements

  • Active and unrestricted Missouri medical license (or ability to obtain)
  • Commitment to high-touch, relationship-centered primary care
  • Ability to work autonomously while collaborating within an interdisciplinary framework

Responsibilities

  • Provide comprehensive primary care through in-home and senior-community visits.
  • Serve as a Bloom ambassador in the St. Louis market, helping build brand presence and supporting market development strategies.
  • Conduct market analysis and engage with local care partners to understand the community landscape and identify opportunities for growth.
  • Manage and grow an empaneled patient population, typically completing 10 patient visits per day.
  • Build trusted relationships with assisted living communities, hospitals, clinics, and community organizations to strengthen care coordination.
  • Collaborate closely with an interdisciplinary team—including specialists, pharmacists, care managers, and social workers—to address complex patient needs.
  • Utilize population health data and condition-specific care pathways to improve outcomes.
  • Participate in ongoing internal education and clinical development programs.
  • Travel occasionally to Denver for onboarding and ongoing training.
