Parkview Physicians Group is currently seeking experienced Board Eligible / Board Certified Family Medicine or Internal Medicine physicians to join our Transitional Care Clinic team in Fort Wayne, Indiana. This is a part-time role requiring 20 hours per week, including rounds Monday through Friday. The position involves travel by conducting daily rounding on Parkview patients in high performing network skilled nursing facilities. Nurse practitioners cover call; the physician would need to be available as a backup by phone.
