About The Position

Sutter Medical Group is seeking a dedicated and compassionate Skilled Nursing Facility Physician to join our post-acute care team. This position focuses on providing high-quality, patient-centered medical care to patients residing in skilled nursing and rehabilitation facilities. The ideal candidate will have experience managing medically complex, post-acute, and long-term care patients, with a strong commitment to continuity of care, interdisciplinary collaboration, and optimizing patient outcomes during transitions of care. Responsibilities: Provide comprehensive medical assessments and ongoing management for patients admitted to skilled nursing facilities for post-acute, rehabilitative, and long-term care. Manage and coordinate treatment for patients with multiple chronic illnesses, acute conditions, and complex medication regimens. Conduct daily or scheduled rounds in assigned SNFs, ensuring timely evaluation and management of acute changes in condition. Collaborate closely with facility staff, nursing teams, rehabilitation specialists, social workers, and case managers to deliver coordinated, patient-centered care. Communicate effectively with families, caregivers, and facility staff regarding patient goals, treatment plans, and care transitions. Ensure compliance with all SNF regulations, documentation standards, and quality measures, including participation in care plan meetings and facility quality improvement initiatives. Partner with hospitalists, primary care physicians, and specialists to support smooth transitions of care and reduce hospital readmissions. Participate in quality improvement, population health, and value-based care initiatives related to post-acute and long-term care. Maintain accurate and timely documentation in the electronic medical record (EMR) system in accordance with Sutter Medical Group and regulatory requirements.

Requirements

  • Medical degree (MD or DO) from an accredited school of medicine.
  • Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification preferred).
  • Active and unrestricted medical license in California.
  • Demonstrated experience in post-acute, skilled nursing, long-term care, or geriatric settings.
  • Strong clinical skills in managing complex, multi-morbid patients.
  • Excellent communication and interpersonal skills, with a patient- and team-centered approach.
  • Ability to work effectively in an interdisciplinary, facility-based environment.
  • Proficiency in electronic medical records (EMR) systems.

Nice To Haves

  • Fellowship training in Geriatric Medicine or experience in post-acute/rehabilitation medicine.
  • Knowledge of CMS regulations, SNF billing and documentation requirements, and quality metrics.
  • Experience participating in value-based care or population health programs.
