As a Primary Care Physician at CenterWell Senior Primary Care, you'll be part of a collaborative team. This team includes APP's, Pharmacists, Care Coaches, Behavioral Health Specialists, and more—all working together to deliver personalized, high-quality care. Reporting to our Associate Medical Director, you will be part of a team dedicated to supporting and empowering clinicians to do what they love. Description Your Role & Responsibilities Provide comprehensive Primary Care to seniors Collaborate daily with interdisciplinary team. Document in Athena EMR. Participate in on-call rotation (limited) Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time. You will work on-site, in an outpatient office setting Use your skills to make an impact

Requirements

• Graduate of accredited MD or DO program from an accredited university
• Board Certification or Eligible (ABMS or AOA) in Family Medicine, Internal Medicine or Geriatric Medicine
• Current and unrestricted medical license or willing to obtain a medical license in state of practice
• Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients

Nice To Haves

• Active and unrestricted DEA license
• Medicare and Medicaid Provider Number
• 2+ years of experience
• 2+ years of experience with Medicare Advantage Patients
• Knowledge of Medicare standards
• Knowledge of HEDIS quality indicators
