Essential Job Functions: Located at AMMC Full Time Demonstrates critical thinking, professionalism, and competency in the RN Role. Clinical oversight and support of Licensed Practical Nurse (LPN) and Medical Assistants (MA) in the clinic(s). Nursing dotted line to Chief Nursing Officer. Collaborates effectively with all members of the care team to ensure excellent patient care. Provides telephone in in-person triage and patient care according to established practice protocol and scope of practice. Engages patients in education regarding chronic illnesses such as diabetes, chronic obstructive pulmonary disease, hypertension, cardiac disease, cancer and chronic pain. Provides telephone care, follow-up and consultation for transitional care management, medication management, INR management, et cetera. Determines the need for and initiates consultations with other services and healthcare providers and coordinates services and care with the patient to ensure continuity and follow up. Documents all care and pertinent information in the patient's medical record accurately and timely. Participates in program development and quality initiatives related to improving patient care, health, and safety. Stays informed of necessary information by attending required meetings and reading correspondence such as emails, memos, newsletters, and policies in a timely manner. Demonstrates a clear understanding of, and commitment to, DECH/CCH mission, vision,and values. Supports and advocates for Evidence Based Medicine and Just Culture. Other duties as assigned by manager. Quality Improvement: Maintains current knowledge of the hospital-wide Quality Improvement Program. Collaborates with management to improve the effectiveness of the department through the Quality Improvement plans and processes. Participates in the departmental and hospital-wide Quality Improvement activities as directed by the manager. DECH is an equal opportunity employer.
