Requirements
- Must be able to take in-house call during weekdays and weekends
- Must have active OR License
- Must have active OR DEA
- Must have full-time availability
- Temp privileges (YES WITH CLEAN FILE ONLY)
- Availability 1st possible date they would be available?
- How many shifts would they be able for per month?
- What month is their schedule completely open?
- Any shift preferences (Day, Night, Blocksetc)?
- Any dates in the next 3-6 months they cant work?
- Any Holidays in the next 3-6 months they would be open to work.?
- Required Certificates: ABLS ACLS ATLS BLS PALS
- Shift timing: 7p-7a shifts, 4 shift block minimum
- Coverage Type: No calls
