This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. This position is authorized to provide the right candidate with the Education Debt Reduction Program (EDRP) to pay up to $200,000 in qualifying loans!! In addition, salary can go above the maximum amount shown if candidate meets annual performance metrics. Duties and Responsibilities: Administrative activities, including performance improvement activities, research, and non-clinical tasks as assigned Exhibits leadership in initializing changes in patient care programs based on current concepts and findings from research and studies Stays informed on developments in the management and treatment of diseases and illness through continuing medical education Responsible for managing and overseeing hemodialysis patients Responsible for completing telehealth visits, e-consults, as well as interfacility consults in appropriate the patient population Responsible for providing consultation on various forms of nephrology disease states, both for diagnosis and treatment in the outpatient, inpatient and CLC settings Will practice the full scope of General Nephrology clinical care, to include comprehensive renal/nephrology services and procedures, functioning as an expert for patient care related to acute, chronic and end-stage kidney disease, fluid, electrolyte and acid-based disorders Appropriately documents own interventions and reviews appropriate health team documentation of patient treatment plans and involve the primary care provider or attending physician when indicated to treat changes in the patient's condition Recognizes complex situations that impact patient care and intervenes, using sound judgment, professional attitude and appropriate channels Incumbent is physically able to work 8 hours per day in an outpatient setting. Direct patient care requires standing, sitting and bending on an as needed basis to provide adequate care Participates in establishing policies, procedures and guidelines to ensure adequate, comprehensive services Participates in in-service training programs and maintains competencies Participates in direct patient care utilizing tele health technologies where appropriate Maintains current knowledge of trends in professional issues Evaluates the need for, and provides education to the patient and or their family concerning the patient's treatment and diagnosis Uses professional standards of care and practice to evaluate programs and activities Develops and leads interdisciplinary teams to improve organizational performance Recognizes impediments to health care delivery and uses problem-solving/performance improvement approaches to improve outcomes Participates in performance improvement activities related to the service line quality improvement process Evaluates need and participates in interdisciplinary ad hoc committees/process action teams for constructive problem solving Establishes objective and measurable goals that are appropriate for case load size based on patient population and diagnoses Develops and implements interdisciplinary standards of practice and care Conducts self-review of current practice and seek constructive feedback from peers Facilitates open dialogue among peers, supervisors and staff Communicates effectively with patients, families/significant others and the health team members
