• Fluency in Spanish is required; proficiency in Portuguese is a bonus.

Responsibilities

• Manage a patient panel of a minimum of 1500 patients.
• Provide quality care and patient-centered care coordination.
• Participate in a call schedule, being on call up to two times per quarter.
