Responsibilities
- Provide comprehensive Internal Medicine care for adult patients, managing both acute and chronic conditions.
- Collaborate with a multidisciplinary care team including advanced practitioners, behavioral health, nursing, and population health specialists to deliver coordinated, value-based care.
- Utilize advanced technology and analytics to enhance patient engagement, access, and outcomes.
- Participate in system innovation, quality improvement, and care redesign initiatives.
