About The Position

Corewell Health is seeking a full-time Physician to join the House Calls team in St Joseph, MI. This role focuses on providing primary care services to homebound patients and those in assisted-living and long-term care facilities. The position offers a collaborative environment with a dedicated team of healthcare professionals, including nursing and social workers, to address complex patient needs. The work schedule is Monday to Friday, 8 am to 5 pm, with a guaranteed salary.

Requirements

• Licensed Physician (MD) or Osteopathic Physician (DO) in the State of Michigan upon hire.
• DEA Registration in the State of Michigan upon hire.
• Basic Life Support (BLS) certification from AHA within 120 days of hire.

Nice To Haves

• Experience in home healthcare or geriatric care.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide primary care services to homebound patients.
• Collaborate with nursing, social workers, and spiritual care providers.
• Manage complex, multi-dimensional patient challenges in various settings.
• Utilize Epic & Suncoast EHR for patient documentation and care management.
