About The Position

As a home care hospice p hysician, you will use your compassion , communication skills and deep expertise to build meaningful relationships with patients and families , providing the highest - quality care at the most challenging point in life. You will see the person behind each prognosis and create care plans centered around the patient’s w ishe s, needs, and beliefs , often working in concert with the patient’s primary care physician . Manage approximately 45-60 patients in their home settings via an interdisciplinary team approach . Make visits in-person and via telehealth . Participate in Weekly team IDG meetings. With your strong sense of commitment, y ou will advocate for your patients, discuss ing goals for their care while address ing their hopes and fears. Yo u will live the hospice mission , s eeking teachable moments everywhere, and educat e patients, caregivers, and the communi ties you serve on the benefits of compassionate , dignified end - of - life care.

Requirements

  • VITAS physicians must be licensed to practice medicine or osteopathy in the state in which their program operates
  • Board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or a related specialty
  • Familiar with the principles of hospice
  • Registered with the DEA
  • Commitment, Compassion, and a Can-do Attitude
