About The Position

We are seeking a board-certified or board-eligible Internal Medicine or Family Medicine Physician to provide high-quality medical care to patients in our partner skilled nursing facilities (SNFs). The ideal candidate will manage complex chronic conditions, oversee acute rehabilitation needs, and lead interdisciplinary team meetings. The ideal candidate will manage complex chronic conditions, oversee acute rehabilitation needs, and lead interdisciplinary team meetings. This role requires a proactive approach to geriatric care, focusing on functional optimization, palliative care principles, and facilitating meaningful goals-of-care conversations with patients and their families.

Requirements

  • Education: Medical Degree (MD or DO) from an accredited medical school.
  • Licensure: Active, unrestricted Texas medical license.
  • Specialty: Board Certified/Board Eligible in Internal Medicine or Family Medicine.
  • Experience: Minimum of 2 years of clinical experience, preferably in geriatric care, post-acute care, or long-term care settings.
  • Strong background in palliative care and geriatric assessment.

Nice To Haves

  • Certified Medical Director (CMD) designation.
  • Experience with nursing home management, quality assurance, and performance improvement (QAPI) processes.

Responsibilities

  • Clinical Rounding: Perform comprehensive patient assessments, diagnosis, and treatment planning within assigned skilled nursing facilities.
  • Chronic & Acute Management: Manage complex geriatric syndromes, polypharmacy, and acute post-hospitalization rehabilitation needs.
  • APRN Supervision & Collaboration: Supervise and collaborate with Advanced Practice Registered Nurses (APRNs) to manage the care of complex, chronically ill patients in both SNF and outpatient settings including assisted living and house calls.
  • Goals of Care: Lead sensitive conversations regarding prognosis, treatment options, and advanced care planning, including palliative care initiatives.
  • Interdisciplinary Collaboration: Work closely with facility nursing staff, therapists, social workers, and families to develop and implement comprehensive care plans.
  • Regulatory Compliance: Ensure compliance with state and federal regulations regarding nursing home care, documentation, and quality metrics (e.g., CMS guidelines).
  • “On Call” responsibilities – approximately 7 weeks/year. This includes one holiday/year.
  • Participation in weekly clinical conference calls
  • Maintain Regular Faculty status in the Division of Geriatrics and Palliative Medicine at UT Dell Medical School
  • Maintain full privileges at Ascension Seton hospitals (or St. David’s hospital)
