The Physician - Geriatrics Transitional Care Program Medical Director position is a full-time role within the Department of Veterans Affairs, specifically in the Veterans Health Administration. This role involves practicing in various settings, including inpatient, outpatient, and telehealth modalities, and encompasses all patient demographics. The Richmond VAMC is seeking a Geriatric physician to lead the transitional care program, which has been operational for approximately 20 years. The position offers opportunities for professional growth, work-life balance, and participation in quality improvement projects. The incumbent will collaborate with an interdisciplinary team and provide education to trainees, while also engaging in scholarly activities. The work schedule is Monday to Friday from 8:00 AM to 4:30 PM, with shared on-call responsibilities after hours, on weekends, and federal holidays.

  • Perform day-to-day clinical activities in the Geriatrics Transitional Care Program and geriatric clinics.
  • Collaborate with the interdisciplinary team and stakeholders.
  • Participate in program development, quality improvement, and Systems Redesign projects.
  • Provide leadership to interdisciplinary teams and administrative staff.
  • Supervise clinical staff including nurse navigators and nurse practitioners.
  • Provide collaborative physician coverage in all GEC programs.
  • Complete medical records and encounters timely and accurately.
  • Participate in educational activities for trainees.
  • Assist the service chief with administrative assignments.
  • Participate in Committees, Root Cause Analysis, and Quality Improvement projects.
  • U.S. Citizenship.
  • Degree of doctor of medicine or equivalent from an approved institution.
  • Current, full and unrestricted license to practice medicine or surgery.
  • Completed residency training in an accredited core specialty program.
  • Proficiency in spoken and written English.
  • Board-eligible or board certified in Internal Medicine/Family Medicine.
  • Fellowship training in Geriatrics and/or Hospice & Palliative Medicine.
  • Recent experience in geriatric transitional care programs.
  • Recent experience in academic hospitalist/internal medicine practice.
  • BLS certification.
  • Competitive salary ranging from $265,000 to $335,000 per year.
