About The Position

Amoskeag Health is a Federally Qualified Health Center (FQHC) and progressive, nonprofit primary healthcare organization that celebrates its 30th year in 2023! Our team of healthcare professionals positively impacts the health, well-being and quality of life for those we serve providing comprehensive, integrated, affordable healthcare and education to patients. We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers. The Physician - Family Practice OB sees clients four days per week in an outpatient setting. No weekends or holidays, no call! 34 clinical hours plus 6 admin hours.

Requirements

  • Medical degree (MD or DO) with a valid medical license to practice in the state.
  • Board certification in Family Medicine or equivalent.
  • Experience in outpatient family practice, particularly with OB care.
  • Strong communication skills and ability to work in a team-based environment.
  • Commitment to providing culturally competent care.

Nice To Haves

  • Experience working in a Federally Qualified Health Center (FQHC) or similar healthcare setting.
  • Familiarity with electronic medical records (EMR) systems.
  • Bilingual skills are a plus.

Responsibilities

  • Provide complete, comprehensive, family-oriented outpatient care for clients throughout the life cycle including health examinations and treatment of medical conditions, to include follow-up on any problems detected.
  • Medically manage acute and chronic illness including acute complications and routine follow-up. Develop a plan of care, coordinate care and determine the need for case conference.
  • Make appropriate referrals to both internal disciplines of Amoskeag Health and/or external agencies for additional services required including sub-specialty evaluation, surgical evaluation, social services, nutrition services, substance abuse and mental health counseling.
  • Provide care to clients with acute illness, allowing for same day appointments as necessary.
  • Prescribe medications for clients for treatment of medical conditions, when indicated.
  • Provide appropriate follow-up with assistance of the nursing staff to include telephone/written correspondence, to inform clients of abnormal test results.
  • Provide clinical consultation to physicians and mid-level providers at Amoskeag Health as needed.
  • Responsible for accurate, timely documentation in the client's medical record of client contacts, case planning and client's plan of care.
  • Provide clear written and oral communication to both Amoskeag Health clients and employees.
  • Participate in an on-call system to provide Amoskeag Health clients access to health care services.
  • Provide in-hospital care for Amoskeag Health clients when appropriate for treatment of medical conditions.
  • Participate in various administrative/clinical staff meetings including Prenatal Case Conferences, Medical Advisory Board, QI Committee, and general Amoskeag Health meetings.
  • Attend mandatory safety-related and infection control in-services as designated by the Senior Management Team.
  • Assist with the development of medical/clinical policies and protocols for clients at various ages of the life cycle, as needed.
  • Become familiar with local agencies to increase the ease of and assure good communication.
  • Represent Amoskeag Health at various national, regional and local conferences.
  • Maintain sensitivity to cultural diversity of population being served and age-related competence.
