Requirements

  • Graduate of an accredited college of medicine with a valid unrestricted license to practice issued by the Arizona Medical Board.
  • Must be registered with the U.S. Department of Justice Drug Enforcement Administration and maintain a valid unrestricted controlled substance registration certificate.
  • Valid driver’s license and reliable transportation for travel between locations.
