About The Position
Theoria Medical is seeking a part-time provider responsible for rounding and managing the clinical care of patients at skilled nursing facilities (SNFs). The provider will deliver high-quality, patient-centered primary care services, ensuring timely documentation of all patient encounters. This role requires daily rounding, prescribing medications, and being available for consultations with nursing staff. The position is in-person, with shifts primarily from 7 AM to 1 PM local time, Monday through Friday, and offers flexibility within reason. The provider will utilize Theoria’s proprietary electronic health record system, ChartEasy™, and may also work with other facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech.
