About The Position
Theoria Medical is seeking a part-time provider to join our innovative healthcare team. This position involves managing clinical care for residents at skilled nursing facilities (SNFs). The provider will be responsible for daily rounding, prescribing medications, and delivering high-quality, patient-centered primary care services. Documentation of patient encounters is essential and will be conducted using our proprietary electronic health record system, ChartEasy™, as well as other commonly used EHR systems. The role requires reliable transportation and timely attendance for scheduled shifts, which are Monday to Friday from 7 AM to 1 PM local time. This is an in-person position, and while charting can be done off-site, the provider must be available on-site during rounding hours.
