Responsibilities

• Rounding and managing clinical care of the population at assigned SNF.
• Prescribing medications and treatments for residents at SNFs.
• Providing comprehensive primary care services to patients in SNFs.
• Timely documentation of all patient care encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Completing documentation in Theoria's proprietary EHR system, ChartEasy™.
• Ensuring on-time attendance for scheduled shifts.
