Responsibilities

• Rounding and managing clinical care of the population at assigned SNFs.
• Prescribing medications and treatments for residents in SNFs.
• Providing high-quality, patient-centered, comprehensive primary care services.
• Timely documentation of all patient care encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Completing documentation in Theoria's proprietary EHR system, ChartEasy™.
• Ensuring on-time attendance for scheduled shifts.
• Maintaining reliable transportation.
