As our organization continues to grow, we’re excited to welcome a dedicated Nurse Practitioner / Physician Assistant (NP/PA) to join our team. This full-time position (4.5 days per week) offers a meaningful opportunity to provide high-quality care to geriatric patients while floating between prestigious senior living communities, outpatient clinics, and nursing facilities in the Raleigh and Cary areas. Geriatric experience is required.

Requirements

  • Minimum 1 year of recent experience in Internal Medicine & Geriatric care
  • Graduate of an accredited NP or PA program
  • Current APRN or PA licensure
  • Current specialty certification
  • Current BLS and ACLS certifications
  • CNOR/CRNFA certification (if applicable)
  • Eligibility for hospital privileges

Nice To Haves

  • Strong time management and care coordination skills
  • Familiarity with healthcare regulations and systems
  • A passion for working with geriatric patients

Responsibilities

  • Perform age-appropriate histories and physicals for complex acute, critical, and chronic patients
  • Order, interpret, and evaluate diagnostic and therapeutic tests
  • Prescribe pharmacologic and non-pharmacologic treatments
  • Implement interventions to support physiologic stability
  • Assist with surgical care as applicable and within regulations
  • Monitor patient outcomes and adjust care plans as needed
  • Support smooth transitions across care settings (admit, transfer, discharge)
  • Collaborate with multidisciplinary teams and make appropriate referrals
  • Educate patients, families, and staff to support informed decision-making
