About The Position

Sub-Acute, Long-Term Care is a sector of healthcare exploding with potential and work-life balance flexibility (no on call or weekends)! We are seeking a provider for a 4 days per week (32 hours) position split between two centers located in Frederick, MD and Walkersville, MD: Autumn Lake Healthcare at Ballenger Creek and Autumn Lake Healthcare at Glade Valley. This position can be filled by a nurse practitioner or physician assistant. 1 year or more of provider experience is required. The base salary range for this position is $56.73 - $59.13/hour, in addition to our no-cap incentive compensation bonus program. Requirements

The Physician Assistant must be a graduate from a program accredited by the ARC-PA and take the PA National Certifying Exam (PANCE) to use the PAC designation.
PA state licensure required.
NCCPA certification required.
In states where practice allows; PA with an unrestricted professional license and DEA for each state in which they practice.

Nice To Haves

A minimum of two years experience working with adult or geriatric population is preferred.

Responsibilities

The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
The Advanced Practice Provider collaborates on-site with the Center Leadership Team for center specific goals and priorities.
The Advanced Practice Provider collaborates with the Medical Director and/or Attending Physician for patient specific medical care.
The Advanced Practice Provider provides primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center.
The Advanced Practice Provider works collaboratively with Center Leadership to achieve AlignMed Partners mission of improving the lives we touch through the delivery of high quality health care and everyday compassion.
