This is a high-bar role for high-impact clinicians. We are intentionally hiring Physician Assistants who want to build, lead, and grow. Our Mobile Primary Care team delivers care directly into homes, assisted living, and skilled nursing facilities — serving complex senior populations through a value-based, panel-driven model. This role is designed for clinicians who want more than a job. It is structured for providers who can successfully build and manage a senior panel — and grow into future clinical leadership roles. Requirements

Active, unrestricted Physician Assistant (PA-C) or Nurse Practitioner (NP) license
2+ years clinical experience preferred (geriatrics or primary care)
Demonstrated ability to build and manage a patient panel
Leadership potential or prior mentoring experience
Entrepreneurial mindset with strong clinical judgment
