Requirements

Clinical credentials: PA certification with a genuine passion for geriatric medicine (prior geriatric experience preferred but not required)

Responsibilities

Provide comprehensive onsite primary care to seniors in assisted living and senior living communities
Develop and execute personalized, patient-centered care plans that address the whole person
Partner with our Medical Director to refine clinical policies, protocols, and guidelines as our practice evolves
Deliver monthly Chronic Care Management (CCM) visits and oversee CCM documentation
Collaborate with senior living leadership and care teams to optimize care plan execution and patient outcomes
