About The Position
Theoria Medical is seeking a full-time provider to manage clinical care at skilled nursing facilities (SNFs). This position offers a unique opportunity to deliver high-quality, patient-centered primary care services while benefiting from a supportive community and innovative healthcare practices. The role involves daily rounding, prescribing medications, and ensuring timely documentation of patient encounters. Providers will utilize Theoria’s proprietary electronic health record system, ChartEasy™, and will be expected to maintain reliable attendance and transportation for in-person shifts.
