About The Position
Theoria Medical is seeking a full-time provider to join our innovative healthcare team. This position offers a unique opportunity to blend medical excellence with technological advancement, primarily serving the post-acute and primary care sectors. The provider will be responsible for rounding and managing the clinical care of patients at skilled nursing facilities (SNFs), ensuring high-quality, patient-centered care. The role includes daily rounding, prescribing medications, and maintaining timely documentation of patient encounters. The provider will also be available for consultations with facility nursing staff and will utilize Theoria’s proprietary electronic health record (EHR) system, ChartEasy™. This position emphasizes the importance of timely attendance and reliable transportation, as the provider must be on-site for scheduled shifts.
