About The Position
Theoria Medical is seeking a dedicated provider to manage clinical care at skilled nursing facilities (SNFs). This full-time, exempt position offers a unique opportunity to deliver high-quality, patient-centered primary care services. The role involves daily rounding, prescribing medications, and ensuring timely documentation of patient encounters using Theoria’s proprietary electronic health record system, ChartEasy™. Providers will also be available for consultations with facility nursing staff and are expected to maintain punctuality for their scheduled shifts. This position emphasizes meaningful connections with patients and staff, supported by a culture of appreciation and professional growth.
