About The Position
Theoria Medical is leading the charge in healthcare innovation, offering a unique blend of medical excellence and technological advancement, primarily serving the post-acute and primary care sectors. Our extensive network includes multispecialty physician services and RPM, covering skilled nursing facilities across the country. We are scouting for the brightest nurse practitioners and physicians eager to drive change and deliver superior care. This position is full-time and exempt, with a focus on providing high-quality, patient-centered, comprehensive primary care services to patients residing in skilled nursing facilities (SNFs). The provider will be responsible for rounding daily, prescribing medications, and managing the clinical care of the population at the assigned SNF. Timely documentation of all patient care encounters is essential, and the provider will be available for consultations with facility nursing staff.
