About The Position

The Physician Assistant at Oak Street Health is responsible for providing high-quality, value-based primary care to Medicare adults, focusing on keeping them healthy and out of the hospital. This role involves independent patient care, collaboration with a healthcare team, and building meaningful relationships with patients to enhance their understanding and connection to their care plans.

Requirements

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate OR Master of Physician Assistant Studies (MPAS).
  • National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner, Physician Assistant.
  • Active, non-probationary state Nurse Practitioner / Physician Assistant license.
  • Active DEA license.
  • US Work Authorization.
  • Bilingual proficiency in Spanish.

Nice To Haves

  • Experience in primary care: internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
  • Passion for teamwork and collaboration.
  • Desire to be part of an innovative model focused on population health.

Responsibilities

  • Provision of exceptional primary care.
  • Conduct office visits for routine and acute issues.
  • Administer annual wellness visits and health risk assessments with accurate documentation.
  • Care coordination with other providers, specialists, and agencies.
  • Lead population health initiatives in coordination with the care team.
  • Assist the care team with phone triage and outreach.
  • Educate patients on their health conditions and care plans.
  • Participate in Oak Street Health promotional activities.
  • Conduct home visits as needed.
  • Perform other duties as assigned.
